Hành Trình kết nối những Trái tim bắt đầu khởi quay ngày 17 tháng 10, 2008 và lên sóng chính thức ngày 02 tháng 12, 2008. Chương trình được phát sóng vào lúc 22:00 vào các ngày thứ ba trên HTV7 và 21:30 trên HanoiTV. Cùng với HTV7 và HanoiTV, 15 đài truyền hình địa phương sẽ tiếp sóng và phát lại.
