Sookie, Bill and Jason return to a Bon Temps turned upside down by Maryann. Lured to Merlotte’s by Arlene, Sam and Andy find cold comfort in their refuge from a group of bloodthirsty revelers. Bill discovers that traditional vampire techniques don’t work on Maryann; Hoyt and Jessica try to keep a lid on Maxine’s madness; and Sookie tries to push through the darkness consuming Tara. With all hell breaking loose, Jason takes the bull by the horns to rescue Sam, at least for the moment.

Damon and Elena try to help Stefan through a confusing time, which ends up taking Elena on an emotional journey. Caroline makes an effort to know Jesse better.

Elena’s new outlook has everyone concerned, leading Stefan and Damon to agree that going back to the normal routine of high school would be the best thing for her. Caroline is pleasantly surprised when Elena decides to rejoin the cheerleading squad, but her pleasure turns to shock when Elena’s behavior proves dangerous. Not giving up on their search for the cure, Damon and Rebekah work together until his unwanted advice catches her off-guard. Klaus tries to use Hayley to get the information he’s after and makes an intriguing discovery in the process. Meanwhile, a bored Elena throws a wild party and gets into an ugly fight.

The stress of managing the hospital and coming to terms with Ben’s decision to become a Seattle firefighter pushes Bailey to her limits.

Caroline does her best to be there for Stefan and Elena in a tough time. Damon tries to convince Stefan to help him uncover Prof. Shane’s motives, but Stefan is intent on carrying out a different plan with Klaus. At the annual Miss Mystic Falls Pageant, Elena and Caroline give April advice on which dress to wear, while Tyler and Hayley work together on a secret project, and Damon confronts Prof. Shane with his suspicions. Alarmed by the vivid nightmares he’s having, Jeremy turns to Matt for help, but the situation escalates sooner than anyone expected.

Amelia and Koracick tackle a daunting surgery on Catherine while Richard and Jackson hope for the best. Meanwhile, Richard urges Meredith to visit her father, Thatcher, before his time is up.

Sookie joins Alcide at a raucous engagement party for his former fiancee. Bill "procures" dinner for Russell and Lorena. Sam brokers a deal with Tommy and his parents. Andy gets a promotion and draws Jason's attention. Arlene is irked by Jessica's arrival at Merlotte's. Eric is given a deadline to locate Bill. Franklin takes Tara on a road trip.

The present-day murders in Mystic Falls remind Damon of a similar crime spree a century earlier. In flashbacks to 1912, Damon recalls a beautiful vampire, Sage, who showed him a whole new way to exist. Sheriff Forbes warns Damon not to get involved in her investigation, but Damon is convinced she’s going after the wrong suspect. Elena and Matt resort to breaking and entering in their search for evidence to prove Dr. Fell is a murderer. For reasons known only to her, Rebekah, becomes interested in locating one of the town’s oldest landmarks. Finally, Elena comes to a new understanding of the path her life has taken, and Damon makes an emotional confession to Stefan.

Meredith, rattled by the latest events, tries to figure out what to do about Derek. Meanwhile, Owen is shocked when his mother is admitted to the hospital.

April's crisis of faith is challenged by a patient she treats who is a rabbi, and Meredith treats a patient who is a transplant surgeon from another hospital.

Everybody attempts to deals with Meredith's trauma as Maggie and Callie perform a risky, experimental surgery on a teenager. Nathan and Owen continue to be at odds. Maggie and Andrew's relationship continues to progress.

Sookie is warned about the dangers she will have to deal with. Dark memories surface for Sam. Arlene must ask Holly for help while Jason faces unexpected dealings with Tara and Crystal. Jesus' interest in V's qualities is aroused. Arlene seeks out Holly for help. Russell vows to inflict punishment on his enemies. After precautions, Eric satisfies a wish.

April and Andrew consult with a fiery patient who has a giant, inoperable heart tumor. Meanwhile, Alex and Eliza are at odds over the treatment of a young patient, and Stephanie and Ben make decisions that could affect their careers.

Meredith has a court hearing after skipping out on community service, while Bailey and Jo have a patient that Jo realizes is from the treatment facility. Richard and Gemma grab breakfast together and things go awry.

Jackson and April travel to Montana in order to perform a complicated surgery on a young patient, but Jackson’s mind is elsewhere, and April is forced to step up and get him back on track.

“Grey’s Anatomy” picks up where the previous episode of “Station 19” left off as both teams tend to a tense situation with so much at stake. Meanwhile, Jo convinces Hayes to bend the rules on a case, and Maggie and Winston reconnect.

Bailey and Ben face a huge, life-altering decision, while Meredith and several of the doctors reflect on the past.
